Use Page 2 to instruct us on where and when deliveries should be made and page 4 explains our terms of sale agreement. Please note that we can only accept completed applications. For your convenience we have provided a copy of the Connecticut Department of Revenue's Sales & Use Tax Resale Certificate, which is a required part of the credit application. Page 1 of the document outlines the state's regulations for resale certificates for sales tax exemption (Regulation 1) and the required record-keeping practices (Regulation 23). Page 2 is the actual certificate and may be filled out on the computer prior to printing.
